Patient Resources

The faculty and staff of the UCLA Division of Plastic and Reconstructive Surgery are dedicated to protecting your personal information.  In accordance with federal regulations, we ask that you bring the following with you to every visit:

  • A copy of your insurance card showing your policy number, group or certificate number.  If possible, please bring a copy of your insurance billing form.
  • A current government or state issued photo ID (such as a military identification card, a driver's license, passport, border crossing card, etc.), or a school issued photo ID.
  • Your Medicare or Medi-Cal card (if applicable).
  • A complete list of any medications, vitamins, or supplements that you are currently taking.

Prior to the delivery of any care, all patients must complete a New Patient Packet, which includes a Billing Information Sheet and a Medical History Form. Whenever possible, we will mail these forms to you prior to your scheduled visit so that you may complete them at home.

In order to streamline the check-in process, we ask that you arrive for your appointment at least 15 minutes early.  This will allow you time to complete any required paperwork.
