<h2>Microvascular Surgery</h2>

<h3>The field of <strong>microvascular surgery</strong> is relatively new, with the first operation performed in 1973.</h3>
<p><img src="images/Reconstruction/microsurgery_1.jpg" border="0" alt="Microvascular Surgery at UCLA in Los Angeles, California" hspace="7" vspace="2" width="240" height="243" align="right" />After 25 years of development, it now holds a critical position in the care of patients. A free flap is essentially a composite block of tissue transplanted from one part of the body to another distant site where its circulation (artery and vein) is restored utilizing microsurgical techniques.</p>
<p>The ability to close wounds and restore form and function utilizing transplanted tissue has forever changed the modern approach to reconstruction and continues to expand the indications for surgery.</p>
<p>The following free flap procedures are performed at UCLA:</p>
<ul>
<li><strong>Breast</strong>: reconstruction with the free TRAM, superior and inferior gluteal flap, TFL flap, Ruben's flap, gracilis flap</li>
<li><strong>Trunk</strong>: coverage of the abdomen and chest, specifically following large wounds typically associated with radiation therapy and cancer resections</li>
<li><strong>Head and neck</strong>: reconstruction of the mandible, esophagus, scalp and cranial base</li>
<li><strong>Lower extremity</strong>: free flap coverage of traumatic wounds and post-oncologic resections, salvage of amputation stumps, bony reconstruction, coverage of nonhealing lower extremity ulcers secondary to diabetes, arterial or venous insufficiency.</li>
<li><strong>Facial re-animation</strong>: restoration of facial expression following nerve injury / cancer resections</li>
<li><strong>Hand</strong>: wound coverage, digital replacement and reconstruction, functioning muscle transplantation.</li>
</ul>
<p><strong>Free flap surgery</strong> is a complex operation that requires a high degree of surgical expertise and experience. By continued refinement of techniques and a high volume of cases, we have been able to maintain a high rate of success approaching 98% patency rates.</p>
<p>Given the low complication rates and high success rates, this field of surgery has shifted from that of being "experimental" and "procedures of last resort" to routine and first-choice reconstructive techniques.</p>
<p>Continued research in the field of flap pre-fabrication and tissue engineering will continue to expand this surgical arena.</p>

<p><strong>Chemical Peels</strong> are a chemical solution, usually TCA (trichloroacetic acid) which is applied to the skin,causing the skin to blister and peel off over a period of 5-10 days. As the treated skin peels off, new more fresh skin replaces it.Chemical peels are a suitable procedure for facial rejuvenation and diminishing the effects of sun damage.Works best on fair , thin skin with superficial wrinkles.</p>
<p><strong>Dermabrasion</strong> is a scraping of the top layers of the skin.This procedure softens edges of surface irregularities,including fine wrinkles and acne scars,especially around the mouth.The results are permanent, however new wrinkles may form as skin ages.</p>
<p><strong>Restylane and Juvederm Utra </strong>are injectable gels used as soft tissue fillers to treat wrinkles and fill lips. Both are composed of hyaluronic acid a substance found in all living organisms. No allergy skin tests are required.<br />These dermal fillers are often used in conjunction with <strong>Botox</strong> for optimum results, especially for rejuvenation of the upper face, and diminshing the appearance of frown lines. Both products last between 4-6 months.</p>
<p><strong>Juv&eacute;derm Ultra</strong> injectable gel is a gel consisting of hyaluronic acid. Hyaluronic acid is a naturally occurring substance in your skin that helps to add volume and hydration. Therfore no allergy test is necessary before treatment. The smooth consistency gel helps to instantly restore your skin's volume and smooth away facial wrinkles and folds, like your "smile lines" or nasolabial folds-the creases that run from the bottom of your nose to the corners of your mouth. It is also used with much success for lip augmentation A Juv&eacute;derm treatment takes about 20-30 minutes and results are immediate. Plus, there's minimal recovery or downtime.</p>
<p><strong><img src="images/Cosmetic/non_invasive_face.jpg" border="0" alt="Chemical Peels, Dermabrasion, Restylane and Juvederm, Radiesse and Sculptra" hspace="7" vspace="2" width="160" height="149" align="right" />Perlane</strong> is approved the for filling into the deep dermis for the correction of moderate to severe facial folds and wrinkles, such as nasolabial folds. Perlane is the newest member of the Restylane family of products and is made of stabilized hyaluronic acid gel particles. The primary difference between Restylane and Perlane is the larger hyaluronic acid gel particles are designed to provide volume and lifting power for effective deeper wrinkle correction.</p>
<p><strong>Radiesse</strong> fills acne scars and wrinkles,including frown lines and smile lines. It is composed of a synthetic compound. It is does not require allergy testing and can be injected in a simple office procedure.</p>
<p><strong>Sculptra</strong> is a long lasting filler used to fill sunken areas of the face,as well stimulate new collagen growth.It is made from Poly-L-lactic acid.</p>
